Two important resources you missed are the comprehensive Lifecare Advance Directive (see www.LifecareDirectives.com) and the Five Wishes living will see www.agingwithdignity.org/five-wishes.php). Studies demonstrate that the more complete your advance directive the more likely your wishes will be known and honored. Both of these documents expand on the usual content of standard state advance directive forms, and, as the American Bar Association has noted, such directive documents often better capture an individual's personal wishes (see: American Bar Association, Patient Self-Determination Act State Law Guide, pg. 18).
